If you’ve ever sat down to figure out how to pay for home care, you’ve probably felt like you’re drowning in a bowl of alphabet soup. Medicare, Medicaid, SSI, PHC, it’s a lot to take in.
One of the most common questions we hear at Companion Care Home Healthcare is: "Will my insurance cover someone coming to help my mom with her bath and meals?"
The answer depends entirely on which "M" word you’re looking at. In Texas, the rules for Medicare and Medicaid are quite different, especially when it comes to personal care services at home.
In this guide, we’re going to break down the differences in plain English so you can make the best decision for your family. Whether you’re in Houston, Austin, San Antonio, or Dallas, understanding these basics is the first step toward getting the reliable care your loved one deserves.
The "Big Picture" Difference
Think of Medicare and Medicaid like two different types of safety nets.
Medicare is federal health insurance primarily for people 65 and older. It’s mostly focused on medical recovery.
**Medicaid is a state and federal program for people with limited income and resources. In Texas, it is often the primary way families pay for long-term personal care.

Medicare 101: The "Skilled Care" Path

Medicare is great for a lot of things, but it has very specific "fine print" when it comes to home care.
What Medicare Covers
Medicare pays for home health services if they are deemed "medically necessary" and "intermittent." This usually means:
Skilled Nursing: Things like wound care, monitoring a serious illness, or administering injections.
Physical or Occupational Therapy: Helping someone regain movement or learn how to do daily tasks after a surgery or stroke.
Speech-Language Pathology: Assistance with communication or swallowing issues.
The Catch: You Must Be "Homebound"
To get Medicare to pay for any of this, a doctor must certify that you are "homebound." This doesn't mean you can never leave the house, but it does mean that leaving requires a major effort (like needing a wheelchair or a lot of help from another person) and that you only leave for short, infrequent trips like church or doctor appointments.
The Big "No" for Medicare
Here is where most people get tripped up: Medicare does NOT cover personal care services at home if that is the only help you need.
If your loved one just needs help with bathing, dressing, and meal prep, but they don't need a nurse or a therapist, Medicare will generally not pay for it. You can read more about this on the official Medicare.gov home health page.
Medicaid 101: The "Personal Care" Path

This is where Texas Medicaid shines. Unlike Medicare, Medicaid is designed to help people stay in their homes for the long haul by providing personal care services.
Primary Home Care (PHC) in Texas
In the Lone Star State, one of the most popular programs is called Primary Home Care (PHC). This program is specifically designed to provide non-medical "attendant services" to help people with their Activities of Daily Living (ADLs).
What Medicaid (PHC) Covers:
Bathing and Grooming: Help with showers, hair care, and shaving.
Dressing and Toileting: Assistance getting ready for the day and managing bathroom needs.
Meal Preparation: Planning and cooking nutritious meals.
Light Housekeeping: Keeping the living area safe and tidy.
How Do You Qualify?
Eligibility for Texas Medicaid is based on two things: Money and Need.
Financial Eligibility: You generally need to have a limited income and very few assets (usually under $2,000 for a single person). In Texas, if you already receive Supplemental Security Income (SSI) through Social Security, you usually automatically qualify for Medicaid.
Functional Need: A state assessment will determine if you actually need help. In Texas, you usually need a "functional impairment score" of at least 24 and must need help with at least one personal care task.

Still not sure? Here’s a quick rule of thumb:
Pick Medicare if you just came home from the hospital and need a physical therapist to help you walk again for a few weeks.
Pick Medicaid if you are healthy but just can't safely get in and out of the bathtub or cook your own meals anymore, and you meet the income requirements.
Pick Private Pay if you don't qualify for Medicaid but still need that extra hand to stay safe and happy at home.
